The Arizona Department of Health contributed to the project's start-up funding.
A-DEC, Inc. provided clinic schematics and photographs of dental equipment for the project.
Henry Schein, Inc. not only provided a manual that they developed for the
National Network for Oral Health Access, but assisted us in modifying to
the manual to better fit the current project.
Dr. A. Kenneth Bolin, Baylor College of Dentistry, provided the report on a study of FQHC dentists and executive directors.
Dr. Jim Toothaker wrote the section about information systems.
Dr. Murray Rosenthal provided considerable input that helped shape Chapter 3 (Finances).
Valuable technical assistance on the development of the budget
worksheets and financial reports (Chapter 3) was provided by Mr. Arthur
C. Wicinski, MBA (Healthcare Consultant, Mentor, Ohio) and Ms. Fran
Netting, CPA (Columbus, Ohio).
Ms. Vicki Twining, of the Ohio Department of Health did considerable
work on the development of budget worksheets.
Volunteers in Health Care provided partial support for an Advisory Committee
meeting.
